Stockport Together selected as a demonstrator site for empowering people and communities (EPC), new care models

In response to the call from NHS England for Vanguards to step forward as demonstrators of best practice in Empowering People and Communities (EPC), Stockport Together’s exemplary work to date has been recognised as the borough was chosen as a demonstrator site.

Along with a small number of other sites, Stockport will be working intensively with the NHS England EPC Team over the next 18 months to:

  • share our approach and learning
  • continue to support the development of best EPC practice to be adopted within the emerging New Care Model design framework
  • draw on the support of the EPC team to help us go faster in our transformative work.

Population-based prevention is a key element of EPC. This includes developing more resilient and healthier communities, seeking to inform, build skills and confidence in the public to prevent future demand, and adopting more targeted approaches to supported self-care for people with existing long-term health conditions.

Stockport’s ‘call to action’, which is supported by the provider board of the new accountable care trust, offers a template to other Vanguard sites. It recognises that reducing demand in the system requires both different expectations and behaviours of people and a new approach to delivering services.
