The Stockport Together partnership has been successful in its bid to be a Vanguard site.
The bid will mean that the partnership will now have the opportunity to work with NHS England and national partners to co-design and establish the transformation of health and social care in Stockport.
Stockport Together is a partnership including: NHS Stockport Clinical Commissioning Group, Stockport Metropolitan Borough Council, NHS Stockport Foundation Trust and NHS Pennine Care Foundation Trust. Leaders from these organisations have committed to make the partnership work a top priority and are working together to develop an overall strategic plan for the borough to integrate and transform health and social care services. Tailored strategies are being developed across four programme areas:
- Prevention and Empowerment
- Urgent Care
- Proactive Care
- Planned Care
Work will continue over the coming months on developing the design of the new models for service delivery. Communication, consultation and engagement exercises will be undertaken over the forthcoming months to ensure that all employees, stakeholders and the people of Stockport are advised of the changes being proposed and implemented.
